How do I submit my Intent to Enroll form?

Instructions:

  1. Log into your AY account.
  2. You will see DECISION STATUS: (located at the bottom) Click on “Your application decision is now available online”
  3. Select the “electronic enrollment form and survey”.
  4. Complete all required fields and select your enrollment decision.
  5. Click submit, your intent to enroll decision will be updated at this time.
  6. Please note that for deferral requests your department will determine your eligibility to defer.
